Gynecology & Obstetrics

We would like the opportunity to present you with the latest information in Natural Hormone Replacement Therapy. This information is vital for pre and post-menopausal women. Many of our patients have inquired about the use of natural hormone replacement and its benefits vs. commercially available products.

Over the course of a womans lifetime many processes are in effect with regard to hormone levels.

Let's start by giving you a brief idea about what is going on with your estrogen and progesterone levels during a normal menstrual cycle. Typically, you will have a cycle consisting of 28 days. Days 1 to 5 are the days of the actual menstruation. A drop in estrogen and progesterone a few days prior signals the body that you are not pregnant, and you begin menstruation. During days 6 to 14, estrogen levels rise. In days 14 to 28, estrogen and progesterone both rise and then gradually fall in the last few days, and menstruation begins again. Keep in mind that there are other hormones involved in the maturation and release of the egg which affect estrogen and progesterone release.

With age, your body naturally becomes less sensitive to the stimulation of the hormones being released, and the levels of estrogen and progesterone decrease. This is the beginning of perimenopause. Androgens, such as testosterone, also begin to decline, but not as fast as the others. Menopause officially begins when you miss 12 consecutive menstruations.

The following list contains symptoms of menopause experienced by most women.

Sleep Disruption
Loss of recent memory
Fatigue
Vaginal Dryness
Decreased Sex Drive
Irritability
Depression
Nervousness
Fluid Retention
Breast tenderness
Headaches
Hot flashes
Night Sweats
Dry Skin
Hair Loss
Difficulty Concentrating
Difficulty in reaching climax
Arthritis
Bladder Symptoms
Weight Gain

If you choose to take hormones to relieve symptoms, you have several options. There are "conventional" hormones and "natural" hormones. Conventional prescription hormones, are commonly prescribed for hormone replacement in postmenopausal women. However, a common factor among these conventional hormones is that none of them are natural -- none of them are found in your body at anytime during your life cycle. They are all derived from other sources, such as animals, synthetic, or "man-made." This might bring you to question, "Do I want something 'man-made' to treat my menopausal symptoms?" This may explain why some of the effects of menopause continue.

"Bio-identical hormones" are hormones that are identical in chemical structure to the body's hormones. The goal of Natural Hormone Replacement Therapy is to replace the hormones that your body has stopped producing with the exact same hormone. The approach is simple. We know that your body produces certain hormones at different levels. For instance, there are three different estrogens in your body, and they are present proportionally as 10-20% Estradiol, 10-20% Estrone, and 70-80% Estriol. These three hormones have different functions and work in different parts of your body.

Progesterone is also a bio-identical hormone that has several essential roles in the body. It counteracts the estrogens' tendency to induce excess growth of the endometrial lining of the uterus, and it builds new bone tissue, resulting in increased bone mass and density. When combined with estrogen therapy, bioidentical progesterone also helps raise the levels of High-Density Lipoprotein (HDL), or "good cholesterol." .

There are many books available on Natural Hormone Replacement Therapy, including Jonathan Wright's book, "Natural Hormone Replacement Therapy for women over 45." They shed considerable light on the subject.
